A Study on Mitral Commissurotomy


Abstract
Ninety four cases of mitral commissurotomy were done in the Department of Thoracic Surgery,
Seoul National University Hospital and the following observations were made.
(1) There were 59 males and 35 females among whom 6 were under the age of 20 years. (2) Seventeen cases of open and 77 cases of blind mitral commissurotomy were done.
(3) In 58 cases intervals between the onset of symotoms and operation were longer than-5 years. (4) Cardinal syptoms were dyspnea on exertion, palpitation, and hemoptysis, each of which occupied
99%, 48%, and 40% of the cases respectively.
(5) Embolism was noted in 12.3% of normal sinus rhythm and 27% of atrial fibrillation cases. There were 11 cerebral, 2 coronary, 3 peripheral, and 1 superior mesenteric artery embolism cases.
(6) Sixty-six cases of pulmonary hypertension were found by cardiac catheterization among which 8 cases showed more than 90mmHg of pulmonary artery pressure.
(7) Atrial fibrillation was found in 39% of the cases.
(8) Valve calcification and left atrial thrombosis were found in 31.9% and 7.4% of the cases respectively.
(9) mitral Minsufficiency were found In four cases and iatrogenic regurgitation was created in one case. (10) Aortic insufficiency was associatied in one case.
(11) Five cases of embolism, 2 cases of hemothorax, and 3 cases of hemopericardium. were com¡þplicated postoperatively.
(12) There were 4 operative(4. 3%) and 7 postoperative (7. 4%) deaths. Among 77 blind operation cases and 17 open mitral commissurotomy cases there were 6 deaths and 5 deaths respectively, showing
total of 11. 7% mortality.
(13) There were 4 cases of restenosis and 2 cases of reoperation. _(14) _Pulmonary artery sclerosis was found in 60% of the cases. (15) Operative results were good in about 70% of the cases.
